What is Melasma?

Melasma is a common skin condition characterized by brown, tan, or blue-gray patches on the face. Melasma can also appear on parts of the body exposed to the sun, such as the neck and forearms. It is a condition that affects women between 20 and 60 years old.

Since melasma is related to melanin production in the skin, sun exposure causes affected areas to darken and become more pronounced.

What Causes Melasma?

While research is still underway to determine the exact causes of melasma, we have learned that sunlight, pregnancy, and certain medications tend to trigger negative reactions. Tanning beds and thyroid disease are also likely contributors. Genetics also may play a role in contracting melasma as the condition does present itself from one generation to the next.

Treating Melasma

A coordinated effort is required to properly treat patients with melasma.

Sun protection is the most important part of any treatment regimen for melasma. Avoid sun exposure, wear a wide-brimmed hat and sunglasses, and use sunscreen with an SPF of 50 or higher, even on cloudy days. Choose a broad spectrum sunscreen that contains a physical blocking agent like zinc or titanium dioxide and reapply every one to two hours when outdoors.

Topical products such hydroquinone, tretinoin, corticosteroids, antioxidants, azelaic acid, and chemical peels can decrease the production of pigment and reveal a more even skin tone. However, continued use of topical treatments is often essential to prevent the condition from worsening.

Laser resurfacing is a safe and pain-free way to remove several layers of skin, after which the skin begins to produce new cells that eventually cover the treated areas. The result is a smoother surface with little to no scarring or scabbing. New skin cells are also likely to be light on melanin to help prevent future outbreaks of melasma.
